Documentation, coding, and case logging

GURS requires the graduating fellow’s case log in its current category format. The submitted log contains sum totals by category, not a list of individual cases, and excludes cases performed by faculty without the fellow. The fellow logs to those categories from day one and reconciles the log at the end of the fellowship year for annual re-verification.
